Objective: Primary Fallopian Tube Carcinomas (PFTCs) are gynecologic malignancies which mostly present abdominal symptoms or vaginal discharge. However, Malignant Pleural Effusion (MPE) as the initial and sole presentation of PFTC without abdominal symptoms and signs is very rare.
Case report: We report a 68-year old female presented with MPE but her transvaginal ultrasound and Computed Tomography (CT) of abdominal and pelvic organs were negative for any suspicious tumor lesion. By combined immunohistochemistry of pleuracentesis, serum CA-125, and PET-CT narrows the diagnosis to a gynecologic origin. A final diagnosis of primary fallopian tube carcinoma originated from small foci of Serous Tubal Intraepithelial Carcinoma (STIC) was made by a laparoscopic surgery. By the literature review.....

Background: Assess safety, efficacy, and ease of use of a novel haemostatic agent in various procedures in endonasal surgery.
Methods: Prospective multicenter case series. One hundred and sixty-seven consecutive patients undergoing endonasal surgery including turbinate reduction, septoplasty and FESS procedures received RADA16 2.5% (Pura Stat®), a novel synthetic peptide solution at the end of surgery to secure haemostasis. Ease of use and efficacy were assessed intraoperatively and at follow up.
Results: Pura Stat® was found to be ready to use, easy to apply and fast to achieve haemostasis. The transparency of the product makes its application to bleeding surfaces easier than alternative bulky and opaque products. It appears to be as effective for haemostasis as other endonasal products with haemostasis efficacy of 98.2%. The use o.....

Although many ingrown toenail orthosis therapies have been attempted in the past, it is difficult to treat ingrown toenails when the nail is short, and granulation is present. Using the SH (Spring Hook) wire, efficient and painless correction of such toenails is possible. 3D application using SH wire enables remote correction and facilitates painless treatment. I would like to show the cases and introduce the actual situation......

Vascular anomalies are a heterogeneous group of vascular development disorders that include two broad categories: vascular
tumors and vascular malformations. Sirolimus and propranolol have been selectively used to some vascular anomalies in case
of accurate diagnosis. Recently, in the article entitled “Antiproliferative therapy with sirolimus and propranolol for congenital
vascular anomalies in newborns (Case reports)” Cîrstoveanu et al. [1] reported four neonates with vascular anomalies treated
with sirolimus and propranolol. There are some controversies regarding the diagnosis and management in the reported cases.
In the article, the authors routinely used propranolol to manage lymphatic malformation, congenital hemangioma, kaposiform
hema.....

Vermiform appendix has surgical importance as it tends to undergo inflammation and cause acute pain. Acute appendicitis is on top of the list producer of acute abdomen in adolescents &young adults, and appendectomy tops the most frequently performed surgical procedure by any surgeon [1]. Epidemiologic studies have showed that approximately 50% (13-77%) of the population will have appendicitis in their life time Butt MQ, et al. [7,8-11], with the peak incidence is in teens 20s, while it’s rare in infancy; risk of acute appendicitis decreases after middle age. Incidence is equal among males and females before puberty and increases to 3:2 at age 25, thereafter incidence declines in males. No cause is yet confirmed but the factors responsible can be decreased dietary fibers and increased consumption of refined carbohydrates, because inciden.....

Non-Hodgkin lymphomas consist of hematologic malignancies of various origins, such as B cell progenitors, T cell progenitors, mature B cells, mature T cells, and, rarely, natural killer cells (NK) [1]. Anaplastic large cell lymphoma (ALCL) is a lymphoid neoplasm that develops from T cells or NK cells. ALCL comprises approximately two percent of all Non-Hodgkin lymphomas [2] and accounts for 16 percent of T cell lymphomas [3]. ALCL occurs as both a primary cutaneous and primary systemic variant [3]. While morphologically alike, these forms are distinct entities of ALCL. Primary cutaneous ALCL appears on the skin of patients in the form of raised lesions [3]. As the name suggests, primary systemic ALCL presents as an extracutaneous disease. Patients with systemic ALCL are categorized as either anaplastic lymphoma kinase (ALK) positive or ne.....

Descending aortic pathologies carries a high mortality risk due to the location of the aorta and the difficulties of repair. An 83 years old male patient had been admitted via an emergency room with a chief complaint of upper abdominal pain and difficulty of breathing. Investigations showed a huge right hemi-thoracic hematoma associated with extravasation suggesting perforation of the distal descending aorta. TEVAR was planned but he was not suitable for TEVAR. Replacement of the descending aorta was
performed successfully under general anesthesia and the use of a shunt from the proximal descending aorta to the abdominal aorta......

Cutaneous metastasis (plural ‘metastases’) refers to growth of cancer cells in the skin originating from internal cancer, develops after the initial diagnosis of primary internal malignancy (such as breast, lung, melanoma, colon, stomach cancer, uterus, kidney and late in the course of disease. Many women diagnosed with breast cancer will achieve cure with surgery followed by adjuvant
chemotherapy, hormonal therapy, or radiation therapy; however, some breast cancer survivors will develop locally recurrent disease. Skin metastasis are one of the most distressing presentations of locally recurrent breast cancer. The presence of skin metastasis signifies widespread systemic disease and poor prognosis [1]. Assessment of cutaneous metastatic disease after mastectomy can
be perplexing because the clinical presentation appears similar to o.....

Patient of 65 years consultant for dry cough evolving for 1 year. The clinical examination found a dullness of the right hemi thorax. Chest x-ray objected to a right lower lobar opacity. The scanner showed a straight basal latero mass, connecting at an obtuse angle with the wall. This mass was tissue-enhancing heterogeneously after injection without net individualization of infiltration or invasion of adjacent structures. On the abdominal cuts there was a right adrenal nodule whose Wash out confirmed its adenoma some nature. There are no other remote lesions. A scan-mistbiopsy with anatomopathological and immunohistochemical study were made thus retaining the diagnosis of pleural solitary fibrous tumor. Pleural fibroid is rare. It is most often asymptomatic and accidental radiological discovery......

Background: This study was planned to compare and evaluate the surgical technique of linear plication and external wrapping (LPEW) to graft replacement (GR) in tubular ascending aortic aneurysms. So, we sought to perform this study of those two surgical techniques to see the possibility to consider LPEW as an alternative surgical technique to graft replacement in well-selected patients with certain circumstances.
Methods and Materials: This is a retrospective study. 65 patients were included in this study and divided into two groups. Group 1 contained 66.2% (n=43) of the patients and were operated on with LPEW while group 2 contained 33.8% (n=22), and were operated on with GR. None of our patients had dissection or aortic root dilatation or had any connective tissue disease. All the aneurysm’s size was less than 60 mm. The two types of.....

Errors come into question in strategy of medicine. An aim of medical measures is an achievement of health, and she engages in illness. In addition, the modern paradigm of medicine does not take into account fundamental position of biology that a human organism is the open thermodynamics system that submits to all laws of thermodynamics. Thus, energy potential of the biosystem is essence of physical health and his foundation, and people fall ill and prematurely die from the loss of health (energy potential). The problem of forming of new paradigm of health protection becomes actual......

Worked out the total to long-term research of author. A human organism is examined as an open thermodynamics system. Her stability at influence of un favorable factors of environment is determined by potential of her energy. The index of this energy is maximal aerobic power (maximal consumption of oxygen). It is an index in the ml/of min/mass of body can be considered quantitative description of somatic health. Having a quantitative index, it is possible to manage a health. The phenomenon of «safe» health level is first described, endogenous risk and disease factors do not register oneself higher than that. Him quantitative description is given to. On leaving of individual from the «safe zone» of health the phenomenon of «self-development» of pathological process is formed. It is necessary in addition to «industry of illness» (est.....

From the point of view, cognitive and psychological, the concept of the word fear means a kind of specific disturbance or not, faced with an idea or not, that is exposed to some type of danger, be it real or imaginary. Fear from a psychophysiological point of view, presents a state of apprehension, of attention, hoping that something bad is going to happen in a certain time space [1]. These cognitive definitions define that, fear is a sensation, and is linked to a state in which the organism is on alert, with or without the exposure of a possible threat [1]. From the point of view of survival, or of the defense mechanisms, whether of conscience or not, fear is extremely important for the maintenance of balance and human survival......

Anaplastic lymphoma kinase (ALK)-positive disease is characterized by the presence of ALK gene rearrangements that encode driver fusion oncoproteins. Echinoderm microtubule-associated protein-like 4 gene (EML4)-ALK fusion is regarded as the most common type and is reported in 2 to 7% of patients with advanced non–small cell lung cancers (NSCLCs). Striatin (STRN)-ALK is a novel ALK fusion partner in NSCLC and is considered sensitive to targeted therapy. However, there was no study regarding effective therapy for EML4-ALK and STRN-ALK double fusion variants in epidermal growth factor receptor (EGFR)-resistant mutant lung cancer. TP53, RB1, and EGFR exon 21 L858R were found in tumor tissues and plasma from patients with capture-based nextgeneration sequencing (NGS). After three months of gefitinib treatment, an NGS of plasma circulating tu.....
